CABHI recognizes the barriers that healthcare organizations (e.g., long-term care homes, retirement homes, hospitals, home care and community-based settings) face when adopting innovations. The Discover + Adopt (D+A) program provides these organizations with funding, training, and skill-building so that they can test and adopt innovations (e.g., products, programs, or services) that improve the quality of care and quality of life of those they serve.

Program participants will receive funding in stages throughout the project duration, based on the completion of agreed-upon milestones.
Program participants can utilize funding and capacity building support to:
$50,000 CAD
In addition to funding, program participants will gain access to:
- a learning series with practical tools and guidance on how to get ready for implementation, evaluation, and plan for sustainability
- 1:1 coaching sessions to help navigate implementation challenges
- peer-to-peer networking opportunities
- opportunities to engage in knowledge mobilization events and conferences only for post-implementation project

Eligibility Criteria
Applicant Eligibility:
- The applicant must be an organization that provides care to older adults (e.g., long-term care home, retirement home, hospital, home care and/or community-based organization).
- The applicant must be located in Canada.
- If the applicant is a research or academic institution, they must be affiliated with a healthcare delivery organization that provides care to older adults, where the solution will be tested and implemented.
- The applicant must have staff members who can participate in all program activities (e.g., learning series, coaching update, planning, etc.)
- The applicant must have leadership support to participate.
- Applications must identify a project lead who will be responsible for overseeing the project and provide regular updates to CABHI.
- Project leads must be employed at the organization and are typically individuals in the organization who lead programs and initiatives, including but not limited to, executive leaders, directors of care, clinical managers, operations managers, program mangers, quality improvement specialists, innovation leads etc.
Project Eligibility:
- The project must identify a pain point that is achievable and within program scope.
- The program requires a co-investment model, aligning with expectations for a matching contribution to CABHI’s funding in the form of cash, in-kind contributions, or a combination of both.
- Projects and associated budgets are expected to adhere to milestones and progress tracking with a focus on sustainability and capacity building objectives.
- The project application and budget must adhere to the Eligible Expense Guideline and Matching Funds Guideline (below).
Application Eligibility:
- The application must be written and submitted by the applicant or a designated team member. The use of generative AI tools (e.g., ChatGPT, automated writing software) for responses is not permitted.
- Only one application per project team may be submitted however; multiple projects from different teams within the organization may be submitted.
