The Centre for Aging + Brain Health Innovation is seeking trailblazing solutions on behalf of healthcare organizations participating in Discover + Adopt.

Please note:
- Intake forms will take approximately 20 mins to complete and can be completed in English or French.
- A ‘solution’ can be a product, program, or service, and is not limited to technological products.
- We welcome all innovators to apply, but due to high volume of applicants, only select innovators will be contacted by the host organization to move forward in the interview and selection process.
A. Cognitive and Mental Health
- We are seeking a solution that encourages social engagement and independent leisure opportunities for residents with cognitive and physical limitations.
- The solution should be easy to use, safe, personalized, and accessible for older adults and care partners, fostering a supportive environment.
- We are seeking a solution to improve staff capacity to communicate effectively with clients with declining cognitive abilities and/or differing communication abilities.
- The solution should be accessible, easy to use, provide staff with tailored communication strategies and promote client independence.
- We are seeking a solution to help staff identify residents at risk for mental health concerns and offer holistic strategies for prevention and support ongoing therapy options that are accessible and feasible within a continuing care context.
- The solution should enhance the accessibility, quality, and delivery of mental health support and services for residents in a continuing care setting.
- We are seeking a solution to address the unmet needs of individuals experiencing dementia or cognitive decline, while also expanding caregiver respite in local community settings.
- The solution should support clients to age-in-place at home within the community longer.
B. Quality of Care and Healthcare Delivery
- We are seeking a solution that fosters a more compassionate and personalized communication experience among the three crucial components in our care delivery triangle: (1) Care Planner/Office, (2) Care Team (field staff members), and (3) Client/Client families.
- The solution should enhance the emotional and relational aspects of communication while maintaining PIPEDA compliance.
- We are seeking a solution to enhance clinical skills training for nurses caring for residents in long-term care and retirement homes.
- The solution should be able to provide the reliable and consistent assessment and training of hands-on clinical skills for geographically dispersed nurses, ensuring residents receive optimal clinical care regardless of the nurses’ locations
- We are seeking a predictive technological solution to proactively identify when ALC patient(s) will be living with chronic obstructive pulmonary disease (COPD) and chronic heart failure (CHF).
- The solution needs to provide push notification to care teams to prepare staff/patients/caregivers for transition including specific patient strategies to support safe discharge.
C. Quality of Life and Client Experience
- We are seeking a solution to help staff in providing better care related to minimizing pressure injuries and falls among residents.
- The solution should ideally provide alerts and notifications to prioritize and inform workflows and enhance the overall safety and well-being of residents.
- We are seeking a solution to accurately assess pain in long-term care residents, including those with cognitive impairment and varied communication abilities.
- The solution should provide reliable information to staff, enabling them to take appropriate actions based on the assessed pain levels, enhancing overall care quality.
- We are seeking a solution to build and improve our capacity to utilize electronic health systems to allow our staff the ability to input, manage, and export data in an automated and timely fashion.
- The solution should help us support data-driven and informed clinical decision making.
- We are seeking a solution to support staff in handling and transferring complex patients with bariatric care needs.
- The solution should reduce the physical demands and risks associated with these tasks for staff, while maintaining comfort, dignity, and safety for patients.
D. Clinical Skills Training
- We are seeking a solution to support patient lateral transfers in hospital.
- The solution should be safe for staff and patients, and ensure a positive and comfortable experience.
- We are seeking a solution to improve LTC residents’ satisfaction with mealtime delivery while improving the efficiency of mealtime delivery as perceived by staff.
- The solution should focus on food temperature and socialization.
- We are seeking a solution that supports resident shower experience in long-term care facilities.
- The solution should support safe and dignified bathing routine, while managing resistive or aggressive behaviours for residents with neurocognitive challenges.
E. Mobility and Physical Activity
- We are seeking a solution to increase the intensity of patients’ physical activity during inpatient rehabilitation through heightened social engagement, competition, and convenience.
- The solution should motivate patients and ideally decrease resource demands on staff.
- We are seeking a solution that will improve independent mobility of LTC residents within their communities.
- The solution should enable residents to move freely and safely, facilitate opportunities for social interaction and meaningful daily experiences, and take into account responsive behaviours.
